The popular ilustration the many of us old farts have seen is in Christopher's Illustrated History of Magic, but it does not show the method. I do recall one similar illustration with the critical areas highlighted. I just can't put my finger on that source right now.
But... do a Google image search for pole levitation illusion, and you'll turn over a few nice rocks.

Hay suggests that levitations in general began with that East Indian trick of the guy sitting cross legged with his elbow alone supported by a stick.
He goes on, "The history of improvements on the effect have been a history of efforts to do away with the supporting stick."
