It all depends on what you want, how you plan to use them and how many of them you need. If you need custom gaffs on playing card stock in high quantities, you should go to someone who prints gaffed cards. My choice in this would be Card Shark.
If you are in the US and you MUST HAVE Bicycle gaffs done on Bicycle stock, such as their proprietary card faces, then Gary Plants would be my maker of choice. I get a lot of stuff from Card Shark, though. His Heirloom decks are my choice for character work.
