The Patrick McCullagh (Pat-Trick) DVD - Punishment includes a one cup routine. Even though this DVD was released in 2007, I have yet to find a video of the performance of this routine.
The description from the DVD reads: Single Cup and Balls - A very workable variation of the classic cups and balls using a single cup and ball bearings with large metal balls for the final loads. Ideal for restaurants. http://www.penguinmagic.com/product.php?ID=S9145

It uses One cup, 2 small metal balls and 2 large metal balls
Available for hangings, funerals or any special occasion.
