It's a Lippincott Box (small) and a Watch Box (large). The Watch Box is described in Hoffmann's "Modern Magic" with a diagram showing how it works. The Lippincott Box works the same way, only it is smaller.
I know this because I am researching the various boxes prior to releasing my own version that uses an ungimmicked box from the craft store. It will probably be in The Wizards' Journal #21 very soon. Meanwhile, there is a debate going on in the Genii Forum about which Lippincott gets the credit, Jack or Malcom.
