As a follow up to a birthday party performance, I would like to send an evaluation report to the parents of the birthday child. If you do this, what are the most effective questions to ask in order to help improve your show?

I send one out with a thank you letter addressed to the child. I would be happy to e-mail you my version if you would like. It would be easier than posting it here. Just send me a PM with your e-mail address and I will get that out to you.

I actually give it to them at the party along with a self stamped addressed envelope. I collect my check and this is what I say to the mom or dad
" I know you guys are busy right but tonight if you could with you and birthay child and family tell me what you thought about the show good or bad. I like to see what my clients are thinking." I get amost of 95% of them back by Tuesday of the next week with more details about the show. Ken |

It is something I've thought about whenever I've changed the show, but never actually done. I'd thought about doing it for the first 10-20 performances of a new show just to guage the reactions. If I did do it like Ken I'd give them out at the party rather than post them.

I use an online form, it can be reached here:

It sounds a good idea, but you should know what your clients think by the amount of repeat work you get. I get a very high percentage of my work from customers who have had me before or seen me elsewhere. That tells me that their happy with what I do and what I charge. No need for any evaluation forms!
