Bill Malone, Daniel Garcia, many more magicians appear to be great at creating a story based on the order the cards come out from a shuffled pack. I've tried doing this, but the story falls apart after a while as there isn't a back-bone to it. So, if anyone else does this totally off the fly, what do you "designate" each card to be, and how do you keep the story going in accordance with the cards?

Well firstly, Sam the Bellhop by Bill Malone is not off the fly at all. The one Daniel Garcia did on the radio show was impromptu except for the first few cards. As long as you have a good outline of the story it's not too difficult to come up with stuff. Just practice at home with a shuffled deck. The story doesn't need to make complete sense. A lot of times it's actually more funny when the cards that come out doesn't match. If you have a difficult time flowing with the stroy from a shuffled deck, then just create a structured routine/story like Sam the Bellhop that comes out the same every time.

Merlin2812, for a nice rendition of Sam the Bellhop, see this clip (from the 80's movie "Brother From Another Planet"):
