Is anyone willing to share their post show feedback form they use with clients? I am putting mine together and I would love to not have to reinvent the wheel If I can avoid it.

Personally I far prefer to send a link to YELP or GigSalad for feedback via email. In today;s social media age, those sites are far more powerful then canned surveys IMO. The YELP reviews will actually drive more business to you which to me is the whole point.
I would maybe distill the best questions from the links Donald provided, but ask folks to review you or leave a testimonial (if they were thrilled) on YELP or GigSalad etc. The trick in this is preparing customers in ADVANCE that they will be getting an email survey for YELP after the show and would they please consider talking briefly about their experience? The prepping in advance - and then perhaps a reminder in person at the END of a show that a survey will be coming - helps to get a much higher response rate and those reviews will get you business. It will also crush some of your local competition who pay no attention to these sites.

Very good point about using those sites and not self serving forms. It has more of an organic feel to it.

One key I can mention is have your rating system 1-5 NOT 1-10
you're more likely to get a 5 this way (of course that is if you were good) but even a 4 then stands out as being a very high mark verses getting a 6 out of 10. i too have been thinking about the web forms idea so glad to hear that it is a good idea.
