I recently saw someone perform an illusion where they put a cardboard box over their head and slid it sideways. Not sure if that is the "Head Off Shoulder Illusion" or not. It looked like an Andrew Mayne creation but I just e-mailed him and he says it is not his.
Can anyone tell me more about the Head Off Shoulder Illusion and wether this sounds like the right name for it?

Not sure if this is what you mean but there is the illusion called Rubberneck.
This is a one person illusion. The performer enters the illusion and is able to twist his head in circles using a crank on the side (like the head twister). He then pulls handles and the head box is pushed up stretching the magician's neck from his body!!! Wonderfully self contained. No black art or mirrors. There are no special lighting requirements and sets up in about 10 minutes. Rubberneck packs into 2 small cases which can be checked as regular baggage with the airlines. The illusion includes an instructional video for performing tips and handling. Rubberneck is an original illusion conceived by Greg Frewin and Nathan Burton. No this one slides along your extended arm. There is a fake face in the box and that is about all there is to the illusion, a cardboard box. I bet it costs a little less than $3000 too. : )

In an old issue of Genii magazine (Volume 48, Number 6 - June 1984) there is a heads off illusion by Paul Osborne called Decapatarium. In this illusion the head is cut with a blade, and while not moved the head is taken off (inside box), very similar to what you are talking about.
I'm not sure if this appears in another Osborne book, but you could do some research to find out, if you can't get hold of the Genii magazine.

Peter Loughran has a version called Head Rush that looks pretty cool.

With facelifter you can't remove the box too much away from the body and have just the neck showing

Looks like a combination of the facelifter box concept and the old disappearing head in paperbag, as when the head moves off the neck, he is showing his back towards audience. very angly

Notice how there's a little jump in the video from when he first puts the box on his head to when he slides it to the side. Obviously, something's missing...

You may be talking about an illusion featured on T.H.E.M. (Totally Hidden Extreme Magic), where the performer's head slides halfway down, near his mid-section. Not sure if that one is on the market yet. However, here is a picture that was taken of my good magic buddy performing Andrew Mayne's "Facelifter" illusion... Cool!

Wow... I remember a lot of people talking down face lifter because of its simplicity. That is a sharp picture there. It looks so real.
