Nathan "Pain" Sparks,

My friend,

This is the first time I have come across this thread, and it really pulled me back in time, and drew a few tears downward from the old "Blue eyes"..

I arrived here at the Café a little bit late around January of 2008, but I am very thankful for it provided the perfect timing and conditions to make your acquaintance.

I am so happy to hear that your wife "Alyssa" is doing so much better, and from one survivor to another, I wanted to let you guys know that I am here for ya, anytime, anything.

Thanks guys...here is a LONG post my wife put up on Facebook this morning:

"My surgeon appointment ended up being a great big bust! (No pun intended!) He ended up being a couple hours late due to emergency surgery (not upset about that) and then wasn't able to get to my mammogram because they switched to a digital system on Monday - so after I talked to him for a short time he wasn't even able to offer advice or anything because he has no clue what's going on! He did say that if he didn't see something obviously awry with the mammogram that he would be ordering an MRI, just to be sure that we're going to biopsy something that needs to be biopsied.

That said - I wanted to share with you some of the high points on the mammmogram report I picked up from the women's center yesterday afternoon.

10/29/2008 report - "The scarring and retraction is extremely minimal and almost invisible and without previous mammography or history I would not actually appreciate that prior lumpectomy had been performed." Good report! Doesn't look like there's been any problem with Lefty.

10/28/09 report - Since previous exam I developed "an incompletely seen at least 20 mm in diameter grouping of linearly oriented branching pleomorphic calcifications consistent with ductal carcinoma in situ (pre-invasive cancer). These are quite similar in appearance to the 10/18/2006 examination and in a similar position." Suspicious for malignancy - biopsy suggested.

So, even though it looks like it is pre-invasive at this point, that could be deceiving as it could have "microinvasions" or it could be making the switch from DCIS to IDC as I'm typing this out! Especially considering how aggressive it was in the first place back in '06 (IDC, grade 3, HER2 positive).

I just want to move quickly so that we can take care of it before it has a chance to do any more harm!!!! I'm not a very patient person anyway - most of you know that! If I have to do this I JUST WANT TO DO IT AND GET IT DONE!!!!!!! That way we can all move on and I'll be a flippin' 2 time survivor. That just seems wrong doesn't it? When are we going to find a cure for breast cancer?! We have many pills that help rich 90 year old men can have sex with their gold digging 20 something wives, but we can't cure breast cancer, yet!? RIDICULOUS!!!!! And, I've heard that Iowa has a high breast cancer rate.....what's up with that?!

Jury is out on whether or not I'll have to have chemo again. Of course, I haven't talked with my oncologist yet, so all I can do is go by what I find on the net and some places say you may not have to have chemo and some places say that chemo will have to happen again, only a different kind than the first time. The latter actually makes sense because if the chemo you had didn't work the first time, why would you repeat a failing treatment?

Hopefully on Monday Dr. Warlick will give me a call and get things rolling - otherwise, I still have an appt. with a breast surgeon in Des Moines Thursday that works with my current plastic surgeon on mastectomy/immediate reconstruction cases. I'm going to talk to my PS about doing a DIEP flap procedure where they take tissue from your abdomen (not muscle like in TRAM flap) and make new boobs. Yes - I'll probably be on my 3rd set by the end of the year....CRAZY!!!!!! I don't think I'll qualify for implant reconstruction again. He was a little concerned about going that route in the first place because of the radiation that I received. I'm a little bummed out that I'll probably not be able to go as big and will have to be on the smaller side again - but hey, they're boobs!!! And Nathan said he doesn't care as long as he gets to keep me around. He really is the sweetest guy on the planet. Has taken care of me through all of this and is a really great dad to Jonah. He really is the best and even though I feel a little unlucky in the health department the last few years, I am the absolutely luckiest gal ever to have found a guy that I can really count on "for better or for worse."

Nathan,

Just now saw this thread.

Both you and your wife are in our prayers. And, anytime you need to vent or express any feelings or thoughts in these regards, just remember that you have a large collective cyber family here at the Café.

Hang in there.

Best,

Mike

Here is the latest news from Alyssa's Facebook:

I heard from the doctor at the breast clinic today.

She consulted with the breast specialist radiologist and Dr. Franks (my plastic surgeon from first recon). She said that the area is very ominous looking and that she feels we should skip the biopsy and go straight to mastectomy. She just said it does NOT look good and that they are all in agreement that is almost definitely cancer again.

So, I am supposed to call her Monday afternoon because they are going to work on getting me scheduled with Dr. Franks, a genetic counselor (to test for BRCA status) and an MRI (to see if any lymph nodes are swollen (indicating that it has spread) and also to see if there are any areas on the right side that look scary). That is supposed to happen at some point next week. After those appointments I will know more. The plan is to get the genetic test results back before we do the surgery because those results will have a huge impact on the surgery I end up having.

Surgery will likely be the week of Thanksgiving or the week after Thanksgiving. She said if everything on the right side looks okay, and BRCA is negative I should be able to just do the left side, but if it's in the lymph nodes, BRCA is positive or something looks iffy on the right side, they're both going.

She's not sure what the treatment will be after surgery, from an oncology standpoint. It depends a lot on if it has spread and what the pathology report says after the pathology comes back after surgery. She said it's basically like starting all over with a brand new cancer. They have to stage it again, etc.

And she thinks I'm too skinny for DIEP flap, but I'm flabbier than she thinks I am. Although, it was nice to hear someone say I'm too skinny for something! Smile

Thanks everybody for your thoughts and prayers! This really is a trying time for our little family. With Jonah's autism and my recurrent cancer it's pretty stressful around here. Keep us in your thoughts!
