Does the dvd include the gimmick or does it just teach how to make one? if the latter, what is the price to make this illusion apart from purchasing the dvd?

It's the DVD only as the gimmick has to be custom made and cannot be pre fabricated.
I have the book version and would say that not counting the parts I scrwed up and had to rebuy, I spent $40 or so.

I also have the book version, and I'd say I spent more like $15-20, but I had some stuff laying around the house that I already had, so this may not be the best comparison.
I use a cloth with a wide hem so I can slip a PVC pipe into it. This cost me the most (like $30-40), to have it custom made for me with blackout material, but I use this cloth for multiple illusions, so this may not count. You can use much cheaper covers such as a bed sheet with a broom stick.

To ask if a specific trick of any sort "fools people" is not asking the right question at all in my opinion.
You need to be asking if the specific presentation for the trick in question is fooling the people who are watching that specific presentation. It is obvious when the answer is no.

I've had my bouts with Andrew lately but I'd stick up for this effect. It can be a great piece of magic if done in the right setting. I have often thought of dong a tribute show. You know the "best off" illusions, performed in the manner of the original artist, and as legit builders will not produce these proprietry props, we would have to produce our own copies up here, giving credit to the original designer, and performer of the effect of course.
Wonder how a show like this would fly (ooops, gave that one away)? I know the lay audience would like it, but even without exposures, I would be hung by the ----- by magicians. LOL Blair Marshall "ShaZzam?!"

I love Bi-Section. As for "does it fool?" I am not so sure that my audience goes home wondering how it is done as much as it is a real shock for people to see it in action. Personally I think most people have some idea of how it is done yet it is so unexpected to see it that it gets a great reaction everytime. I use it as a comedy opener so that I immediatly establish myself as a funny and outrageous entertainer (I hope).

Its definitely shocking. There's the mystery of where in the world did the top half go? and the 'restoration' can be quick, so theres no time gap where a lot of people KNOW there is dirty work being done. The first time I saw this I was like... WOW, I know that didn't just happen, but I felt like my eyes were tricking me..
...for the 51st time... that is NOT my card...
