Why is it that some books and videos go "out of print", "no longer available", etc., when there is still a demand for them?

I am sure there must be a good business reason for it, but it makes no sense to me.

Publishing books is expensive and gets done in runs of 1000, 2000 and maybe 10,000 units.
The single unit/publish on demand technology does not produce the high quality product expected of a hardcover book with photographs and colored illustrations. On the other side, copyright for digital media is problematic.
This leaves the publisher caught in the bind of having demand in small quantities and being unable to publish on demand.
No one but the used book seller is happy about this. -Jonathan
