Does anyone today find that effects along the lines of skewer through tongue are not as effective due to the popularization of body piercings such as tongue piercing? What do you do to counter act this?

Yes. If an effect is going to have a possible and mundane explanation, it is not worth doing.

Being that sideshow performers do this stuff for real, I prefer to leave them to it.

My thoughts are if you can give the illusion that you've pierced your tongue with a metal skewer and then remove it showing no wound or piercing afterward it's definitely worth it! I've performed this many times with great reactions and no, my tongue is NOT pierced.
