As mark said - hats are great.

You could also make dragon or bat wings for boys.

Or theme it like Mark's Pirate hat is really good. Add a parrot, eye patch, sword...
etc

No never think out of the box! Especially in america with all thoes law suits! If you keep teh child in the box (obvisly padded with a good supply of air and pleanty of things todo as long as they wont hurt him) nothing can happen to him and you wont get suied!

I like teh idea of using un inflated balloons as an eye patch - but the americans cant use it .... some other brat at teh party would pull the balloon and snap it on his eye - hurting him and then Alexa would get suied for supplying the balloons, you would be suied for suggesting it, the magic Café would be suied for hosting the suggestion, qualitex (other makes available including correct spellings) would be sued and have to print a warning on every ballloon, Mark would be sued for suggesting a hat and I would be sued for taking it too far.

On balloon Chat, (a British balloon forum), there were photographs of a horse that was wrapped around a kid's waist. The kid held onto the reigns. I thought that it was a stroke of genius. To the best of my knowledge, there are no instructions for it, but if your well versed in balloon sculpting, it wouldn't be too hard to figure out. Combined with a cowboy/cowgirl hat, it might be the answer.

If you poke the kid in the eye first ..... the eye patch is very appropriate.
