I have developed a version of the Psychological Stop card trick, and part of this is to throw the spectator off the scent with a sort of "psychological profiling" presentation. To this end, I present them with a small form to fill in, containing the following ten questions. Do you think I have missed anything?
1. Full name 2. Date of birth 3. Gender. 4. Venison or Guinea-Fowl - please state preference. 5. You are stuck in an elevator with no panic button - do you force the doors or cry for help? 6. On a scale of 1 to 10, how tall would you say you are? 7. How would you describe the atmosphere in this room right now? 8. What is your weekly consumption of fruit and veg? 9. What springs to mind when you hear the word "wobble"? 10. Please state sexual orientation, and any supporting reasons.

Gender should be changed to sex: gender is a characteristic of nouns (in some languages): masculine, feminine, or neuter; sex is a characteristic of plants and animals: male or female.
I'm not sure I'd include #10, and I might add (or change #11 to): paper or plastic? I'd also try to shorten it up a bit: maybe have the "description" questions be multiple-choice. You may find that spectators don't want to spend too much time filling out (filling in?) a form. |

Filling out a form is not entertaining nor is watching somebody do so. If you insist, three questions is more than enough (or just do a ten phase Ambitious Card Routine).
