Based on Tony's nonsensical performance during this effect, I'm guessing he was drugged up an insane amount in order to tolerate the pain of this thing.

I witnessed this in Las Vegas last February right on the street by Mr. Star.
It certainly had the audiences attention and they were cringing.
Hope he makes a mint selling this to Angel or Blaine.
It takes 4 to 6 minutes for the brain to die without oxygen. I can fix anything in that amount of time.
