Marc, you are describing a Lester Lake Extended Blade Guillotine. Depending on your price range the top of the line would be one from Owens, or Wellington.
There are a lot of junk guillotines out there, be careful with what you buy. If you go with one custom built make sure the builder has experience building this one. I've seen several malfunction, it didn't cause injury, but when the blade was removed at the conclusion of the effect not everything went back in place if you can imagine what I mean.

Peter, this happened to two different magicians using equipment from two different manufacturers, both performers just stood in horror with a total look of shock.
This is the true magicians nightmare...

I couldn't even imagine!
The only guilotine horror story I have is a long time ago when I was kid I used to have one of those old Mak Magic French Guilotines. And once some teen beat magazine thingy for our local area was at my show and took several photos of my show and did a little interview with me afterwards. Well when I saw the published article come out...one of the pics captured the guilotine in "Freeze Frame" so to speak showing both blades(1 up 1 below), revealing the entire secret of the illusion for all the readers to see. I can't beleive they published it. It was an honest mistake, but a stupid one on their part. You think they would have realized! After that I made sure that no photography was taken during that illusion again. P.

Thanks Peter for the added info on the Wolf guilotine. Don't imagine I could afford it even if Chance was willing to build one for me.
I've never had any problems with mine. Must say that your horror stories make me want to pack it away and never get it out again. Mine has the stocks to hold the hands which keeps little finger out of the way. That was my greatest fear of using one. Brad
